*Jefferson Lab's 2012 Health Benefits Open Enrollment begins _today_ and
will go through, _Wednesday February 29, 2012_*
* *
* *
_Below are a few reminders concerning the Open Enrollment Process__ _
*Default Enrollment: *If you want to leave your benefit elections as is
and make no changes, you are not required to re-enroll.
**
*Conditions in which you will need to access the Employee Self Service
(ESS) System:*
*1. **To select another medical option outside of the medical plan
default ;*
*2. **To make changes to your current benefit elections;*
*3. **To participate for the first time in a Medical Care or
Dependent Care Flexible Spending Account Plan; ***
*4. **Continue your enrollment in a Medical Care or Dependent Care
Flexible Spending Account***
*Important Things to Remember:*
* *
*Due to Anthem's Medical health plan/product realignment, employees will
automatically default into the most comparable medical plan as indicated
below:*
* *
o *BlueCare Indemnity ? KeyCare PPO 10*
o *KeyCare PPO 15 ? KeyCare PPO 10*
o *Healthkeepers HMO 15 ? Healthkeepers 15 POS *
* *
? *Optima Vantage open access HMO will continue to be offered as is*
? *Health Care Flexible Spending Account (FSA) maximum contribution
changes from $5,000 to $2,500*
? *Short-Term disability premium rates decreased approximately 35 percent*
* *
A matrix is attached that provides a high level overview of the medical
plans and associated cost.
_ Grandfathered Plan Special Enrollment for Adult Children_
As a result of the Health Care Reform you may be eligible to enroll your
adult children up to age 26 on your medical and dental plan. Your
dependent may be added to your health coverage regardless if they reside
with you, their marital status, or financial dependency. However, your
adult children are _NOT eligible_ for coverage on your insurance plan if
they have group sponsored coverage available through their employer.
_Instructions to Log Into the Employee Self Service (ESS) System_
To log into the ESS go to *misportal.jlab.org/time*
<https://misportal.jlab.org/DeltekTC/> or click on the open enrollment
link from the Insight page. To learn more specifics regarding this
year's plan changes and enhancements, please view the 'Summary of
Benefits Matrix' found on the Benefits web page or the attached PDF.
Training on the ESS System as well as an overview of the 2012 Anthem
medical plans and all other sponsored Lab Benefits will be offered at
the following times:

February 17 2:00 - 3:00 p.m. Anthem Representative
Wednesday February 22 3:00 - 4:00 p.m. Treva
