MEDCON 5 User Access Clarification
As a condition for the Lab’s resumption of limited onsite operations, all Users invited to return and approved for on-site access will need to complete the new SAF-003 Training “Working Safely at Jefferson Lab During Elevated MEDCON Levels” and the Lab’s health survey questionnaire before arriving on-site. To ensure users have completed both requirements, all Lab photo badges issued to users have been deactivated.
If you are a User that has been identified by the Physics Division to return during MEDCON 5, you must complete a new registration to request site access<https://urldefense.proofpoint.com/v2/url?u=https-3A__jlab.us11.list-2Dmanage.com_track_click-3Fu-3D1de9c43e6d5e45ff0ecd664cd-26id-3D403f9e50aa-26e-3Da2be82586b&d=DwMFaQ&c=CJqEzB1piLOyyvZjb8YUQw&r=131CRgxL4Epw68sZxTQ8XA&m=nzvUUC39Y0MUNSPjxr-MvnwSPLsHqLRSuOKw9Hnqg50&s=4pKwmRzpeeh09olw9PzHW14FBBlwuRJNKoanl23SCJg&e=> and complete a health survey questionnaire two days prior to your arrival on-site to reactivate your Jefferson Lab photo badge. The SAF-003 training will be included in the Registration Checklist to be completed before arrival onsite. Once an access request is approved, Users will receive an email alerting them their registration requirements have been met and approved. Two days before a User’s scheduled onsite arrival, an email will be sent notifying them to complete and submit the health survey questionnaire. If you have questions regarding the date of your onsite visit, please contact your Jefferson Lab sponsor or host for clarification.
Only Users who certify they have not tested positive for COVID-19 within the past 14 days, have not had symptoms consistent with COVID-19 for the past 14 days, have not had close personal contact with another person who has tested positive within the past 14 days, and have not flown on a commercial aircraft within the past 14 days will be admitted. If answering affirmatively to any of these questions, clearance from Occupational Medicine is required before on-site access and badging is provided.
Please continue to use the COVID-19 Information Portal<https://urldefense.proofpoint.com/v2/url?u=https-3A__jlab.us11.list-2Dmanage.com_track_click-3Fu-3D1de9c43e6d5e45ff0ecd664cd-26id-3D3013685e0d-26e-3Da2be82586b&d=DwMFaQ&c=CJqEzB1piLOyyvZjb8YUQw&r=131CRgxL4Epw68sZxTQ8XA&m=nzvUUC39Y0MUNSPjxr-MvnwSPLsHqLRSuOKw9Hnqg50&s=rJCRUFeOZda9EbgAf5mH78REgmBRMCbmziumrK9zIZM&e=> as your comprehensive resource for all information related to the pandemic and the lab’s status.
