[Dsg-rich] RICH Mirror packaging information

Valery and Marco, 

The mirrors have not been packed yet. We do have the boxes and packing material on hand and will be packing the mirrors tomorrow morning. Once the mirrors are packed, we would need to notify JLab Shipping and Receiving. When would be the deadline the mirrors need to be shipped by? 

In regards to any special documentation from JLab, if it is a domestic shipment, we would need a to fill out just a Shipment Authorization Form. If it is international, we need both a Shipment Authorization Form and a Proforma Invoice for International Shipments. The links to these forms can be found at the link below. The Shipping Authorization Form also notes that additional project-organization approval is needed if shipping costs exceed $1,000.
