[Dsg-rich] Details needed for RICH mirror shipment

Dear Marco, 

The mirrors have been placed inside of the boxes for shipment, however, the padding will be added with the help of Shipping & Receiving after the Shipping Authorization Form has been submitted. 

Looking at the Shipping Authorization Form, we would need to know the following details: 


    1. CMA contact that shipment will be addressed to 
    2. CMA's shipping address 
    3. How the shipment will be payed for (INFN purchase order, JLab purchase order, or other) 
    4. What account the shipment should be billed under

With the above information, we would be able to fill out the form and proceed with the packing process.
