[Dsg-rich] Signature Needed for Authorization for Shipping

Hello Danny, 

This would be the third shipment of these mirrors we have arranged and sent with insurance in 2017. 

The other two were: 
1.) January 19, 2017 Authorization Form #65114 
2.) May 5, 2017 Authorization Form #67390 

These shipments, while arranged by JLab, were paid for by INFN. The new shipment under Shipping Authorization # 69028 will be paid for in the same way. 

Please let us know what other information you need to be able to sign the authorization.

The last word I got was we are not allowed to pay for specific insurance. Before approving you need to coordinate with Christian Whalen and Rhonda Scales (legal)
