[Dsg-rich] shipping authorization form for remaining 4 RICH mirrors from CMA to ECI

For the next shipment of four RICH mirrors from CMA to ECI the mirrors will be packed as follows: 




Handling units: 2 




Unit 1: 

Mirror in unit: 2 

Dimensions: 27" X 29" X 40" 


Weight: ~50 lbs 




Unit 2: 

Mirror in unit: 2 

Dimensions: 27" X 29" X 40" 


Weight: ~50 lbs 




CMA has notified us that they will not have the mirrors ready for pick-up until Monday October 2, 2017. They do not have to ship tomorrow.
