Deficiency Hazard (ODH) Control Program and its associated appendices 
documents:
 

    * * T1 Oxygen Deficiency Hazard Safety Review and its associated Form
    * * T2 ODH Classifications, Engineering and Administrative Control
      Practices, Training, and Medical Exclusions
    * * T4 ODH Emergency Response Procedure

 
The Technical Point-of-Contact for these documents is Dick Owen, Oxygen 
Deficiency Hazard Safety SME, as such he played an integral role in the 
development of this material. Technical Reviews were provided by (in 
alphabetical order): Robert Adams, Engineering; John Kelly, ESH&Q 
Division; Nathaniel Laverdure, Engineering;  Kelly Mahoney, Engineering; 
Mathew Wright, Engineering, Jennifer Williams, ESH&Q Division.
 
These documents were drafted in a response to a concern that ODH 
assessors were not getting adequate information from owners to perform 
an assessment; assessments were not adequately documented; and it was 
difficult to establish controls once an ODH had been introduced into an 
area.
 
It is asked that you provide these documents to affected members of your 
organization for review. Any comments are to be directed to the ES&H 
Manual Editor (at mbailey at jlab.org <mailto:mbailey at jlab.org>) by April 
22, 2011. If you have no input at this time a "no comment" response 
would be appreciated but is not required.
