Attached, please find for your review, draft "ES&H Manual Chapter 3310 
Operational Safety Procedure Program" and its associated appendix:

    * Appendix T1 Operational Safety Procedure and Temporary OSP Procedure
          o Operational Safety Procedure Form

 

The Technical Point-of-Contact for these documents is Harry Fanny, 
Deputy DSO -- Accelerator as such he played and integral role in the 
development of this material.  Technical reviews were provided by Bob 
May, Deputy Director and DSO ESH&Q.  Comments were included as appropriate.

 

These documents were edited per a request made by Mary Logue, Associate 
Director -- ESH&Q to incorporate the Associate Director - ESH&Q into the 
OSP hazard review process; specifically when it relates to work 
activities that cannot be conducted in a manner consistent with ES&H 
Manual requirements as written; or when a new/anticipated/previously 
unrecognized hazard issue is introduces. 

 

*Revision Summary*

    * 3310 and 3310T1 - program and procedure have been streamlined in
      accordance with current ES&H Manual format to include the AD-ESH&Q
      Review.
    * OSP Form -- added checkboxes to the form so owner can denote why
      the OSP was written. This will (hopefully) flag reviewers to any
      additional review requirements.  

 

It is asked that you provide these documents to affected members of your 
organization for review.  Any comments are to be directed to the ES&H 
Manual Editor (mbailey at jlab.org <mailto:mbailey at jlab.org>) by November 
23, 2011.  If you have no input at this time a "no comment" response 
would be appreciated but is not required.
