Attached please find for your review of "ES&H Manual Chapter 6640 
Hearing Conservation Program" and its associated appendices:

    * T1 High Noise Hazard Evaluation and Monitoring;
    * T2 Hearing Personal Protective Equipment - Required and Voluntary
      (including Medical Monitoring); and
    * T3 High Noise Posting Requirements

 

The Technical Point-of-Contact for these documents is Dick Owen, 
Jefferson Lab's, Excessive Noise Subject Matter Expert, as such he 
played an integral role in the development of this material.  Technical 
Reviews were provided by (in alphabetical order): Dr. Smitty Chandler - 
Occupational Medicine; Carroll Jones - Facilities Management; Scott 
Thompson -- Machine Shop Engineering Division; Mark Weihl - Engineering 
Division; and Jennifer Williams - Industrial Hygiene. 

 

These documents were drafted to reflect current laboratory operations.

 

It is asked that you provide these documents to affected members of your 
organization for review.  Any comments are to be directed to the ES&H 
Manual Editor (mbailey at jlab.org <mailto:mbailey at jlab.org>) by April 12, 
2012.  If you have no input at this time a "no comment" response would 
be appreciated but is not required.
