[Evpo] Subcontract PO Accruals - 3rd Request and FINAL Request

Hi Pam,

We’ve already received the accrual forms from Sandesh for PO-C0164 and from Gigi for PO 22-C0008. The only one still outstanding from the list you sent is PO 25-D1331.
 What we need is the accrual form to be filled out for work performed through 11/24/2025 - this is the date to use on the PO Accrual form as well.  I have attached a blank version of the form along with the one Sandesh sent today for your reference what it looks like.

Once you’ve filled it out, please send it back to evpo at jlab.org<mailto:evpo at jlab.org>. If you have nothing to accrue, please email evpo at jlab.org<mailto:evpo at jlab.org> stating you have nothing to accrue for PO# xxx-xxxxxx.

Hi Annie,

After reviewing the spreadsheets attached to your email below, the only Purchase Orders applicable to Physics with remaining balances total $557K (across POs 24-C0164, 25-D1331, and 22-C0008, as shown above).

Before I proceed, I want to confirm I understand your request.

Are you asking whether the remaining work associated with these POs is still outstanding through November 24, 2025, and if so needs to be re-accrued on the November PO Accrual Form? In other words, the Magnet Group must determine whether any portion of the remaining balances represents work that has been completed but not yet invoiced, and if so we must submit the percent-complete forms accordingly?

Please let me know if my interpretation is correct or if you need something different than my understanding.

Submittals were originally requested by COB November 17, 2025 with the second request by Noon Today, November 19, 2025.   It is imperative that you forward November 2025 accruals ASAP.

Please complete the Subcontract Percent Complete Form (PO Accrual Form) for work completed through November 24, 2025- this is the date to use on the PO Accrual form as well. It is important that we receive estimated percent complete forms associated with work performed through this date for which we have a contractual obligation where an invoice has not been received and approved.
