There was a lifting incident earlier this month at SLAC and here are some photos and a sketch of the lift configuration 

Also here is some narrative to go along with the photos, and here a few things of note: 


* The corner softeners (angle iron welded to pipe section) were only 8" in length. 

* They attempted to tethered the corner softeners to the slings using small gauge electrical wire (along with cardboard and duct tape) but those retaining wires broke shortly after the slings started to seat themselves under tension. 

* A better choice of rigging would have been I-beam clamps on the top of the load, but that may have reduced their overhead clearance. 

* The crew responsible for the rigging were representatives of the crane manufacturer, and not the company that operated the mobile crane. 

* Once it was noticed that one of the softeners was no long providing protection between the sling and the top of the load, they started lowering the load. The crane operator stopped that decent for some reason, which caused a slight bounce in the load. This is when the sling failed and the load fell. 

* There hasn't been an explanation provided yet on why the hoist trolley was oriented at the outboard part of the load instead of at the center. 

* I believe Tom said the two chokered slings were applied in the same direction instead of opposing directions. The combination of the last two items may have contributed to the torsional twist that appears in the load.
