On Tuesday, July 25, 2017, a TJSO staff member suspended the work of a 
contractor in CHL due to inadequate fall protection.    Two immediate 
lessons learned arose from this event:

  * If the planned work goes beyond that as described in the approved
    lift plan, then additional planning, hazard analysis, and perhaps
    work control documentation is required.
  * When working from heights, involving the SME in the planning process
    is critical to developing a thorough work plan.


We have determined this to be a Notable Event, and a summary of the 
initial Fact Finding meeting is attached for your information and use.  
Also attached are photos of the work after it was authorized to restart. 
These may make a good tool box discussion.
